Affiliate Information
Anyone providing services to developmentally disabled individuals must “affiliate” with the CDDO in the area. Individuals providing Supportive Home Care, agencies providing Day and Residential Supports and/or Case Management, as well as those providing Assistive Services must maintain their Affiliate Agreement.
