Apply for I/DD Services

The CDDO manages the HCBS waiver for I/DD (Intellectually Disabled/Developmentally Disabled) services for individuals age 5 and older who meet certain qualifications.  The first step is to call and make an appointment to apply for services at 620-221-5404 or 620-441-4504.  Please note there may be a waiting list for funding for services.

In preparing for your initial appointment gather the following documents to bring with you:

• Psychological evaluation (ages 7+)
• Medical examination (if you have a Developmental Disability diagnosis)
• Social Security Card
• Medicaid card/private insurance (neither is required)
• Birth Certificate
• Guardianship papers (as applicable)
• Individualized Education Plan (IEP)

It is not necessary to have all of these to schedule an appointment, but it will speed up the process if you come with all of the above.  The individual who is in need of services must attend the initial appointment.  Call our office at 620-221-5404 or 620-441-5404 if you have any questions.
